Informed consent and acknowledgement
I hereby give my approval for my youth's participation in all activities prepared by Nicola Valley Evangelical Free Church (NVEFC). In exchange for the acceptance, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the NVEFC and all its respective leaders from all liability for injuries to said youth arising out of participating in youth programs.

Off-property events
My youth can attend off-property NVEFC Youth events (i.e. bowling in an evening). One week's notice will be given about details and location unless the event is at Rotary Park beside the church.

Medical emergency
In the event of sickness or medical emergency, I request my youth receive any medical attention or treatment deemed necessary as determined by NVEFC leadership. You will be contacted as soon as possible.

Transport home
I agree to transport my youth home if they disqualify themselves (i.e. possession of tobacco, alcohol, or illegal substances, or unwillingness to obey the youth leader in charge) as determined by the youth leader.

Bounce castle
I agree to the conditions allowing my youth to use the Bounce Castle.

Media release
I agree the NVEFC can post videos / photos online of my youth for the purposes of showing / celebrating what happens in the ministry.
